Assuming that you know nothing, you should post in Service Request Section.
Otherwise, just make it in PHP, form > textarea and then a little code in PHP to retrieve and save the data.
Which language are you using ?

by looking at his title I could guess that he is using HTML. but i do have to agree with you, you should try using PHP, it is much easier to create an interactive website with PHP then with html

I don't really think you understand how PHP works or what it is.
It's just a server-side language that can be used to simply process data or make a webpage dynamic. That's the shortest explanation I can give it.
OP: Make a <form> and have it POST to a .php script.
