Make sure in your vulnerability scanner to have "logs" at orange to purple.. next OP google
vulnerability assessment template ext:doc
The second link " Risk Assessment Template (.doc) " give that a look. I suggest you import results into Excel don't include say software images and all that, just the important stuff.. you can include and I suggest you do include what you used to get your results.
