Summary:
A SpaceX prototype rocket (Starship SN4) has exploded today during a firing test in Texas. This comes 23 hours before SpaceX plans to launch it's first humans into space with their other rocket. This rocket engine that has exploded ("Raptor"), was meant to be a future upgrade for the Falcon 9, which will be used in tomorrow's human launch.

I believe the space shuttle is actually the deadliest space vehicle with the highest mission failure rate.
