If both your password and seed phrase are lost, recovery is nearly impossible without backups.
The best prevention is securing and backing up your seed.

Seeds and phrases are easy to recover, but what most people don’t understand is losing a password to an old wallet.dat file, which is what Litecoin, BTC, Dogecoin and a few others used. There’s also JSON for Ethereum. The only way to truly unlock it if you forget your password is cracking it with Hashcat and a GPU, and you better hope that password isn’t complicated (unless you were very very lucky with a password generator that was weak, like how one person managed to get their bitcoin back with a password that was random by using the timestamp on the computer and the faulty software).

I'll never truly understand how people invest, or have large money invested into crypto.. and then make silly mistakes like not keeping a physical copy of said password / seed phrase somewhere. There are just too many alternatives one can use for keeping a backup of this information.. there should never be a time where you're completely locked out your ish. But yet it happens everyday.
Let alone talk about all the people who end up losing more money, by falling for illegitimate crypto recovery scams. Slow down. Back up your ish. Make a backup, for your backup. Put them in places offline.
