I have experience with using kingphisher, gophish, evilginx, SET, etc. Websites can be setup on VPS, VM, self-hosted, whatever. I can forward ports for you and do any of the technical requirements. Do you know a reputable host that wont openly suspend, I have a modified version of evilginx, and gophish that would need some custom phishlets provided this is something you're willing to undertake, given its complexity I expect to spend a little more.
