they follow the chain. many addresses end up going through a regulated exchange or otherwise. say you send btc to someone that uses coinbase. coinbase (or someone with info from them) says to them "hey who sent u that" and theyre like "o some dude winds on hf"...then feds go to omni "hey man whats this dudes ip addresses" etc etc etc
