He said I was accepted into the group and he said he needed me too fill this form in with google forms [They log answers and give to owner]

Looks fake to me as if there domain had been seized would they host that image on the fbi website not that certain domains hosting?

Fake. Look at the domain's whois, if the FBI really seized it, it would've had FBI's info on the whois.
