You want to populate a MySQL database with NTP server IP addresses/hostnames?

Do you want the attack script written in just PHP (obviously, excluding running an external file)? If so, I don't believe you can - you cannot spoof a UDP source address in PHP (without modifiying the PHP's source code).

So explain what you want; your own website where you can command a DDoS attack?

I mean that I can get you fresh ntp lists, setup attacking scripts.
And that I can setup the PMA exploit
