Pentesting Question

Hello,
I was wondering if anyone out there could explain a good way to get this done.
I'll give an example.

Say you're at a eating establishment or Dr. office etc. and this place you are has a open wifi for its customers to access while they wait or eat or whatever it is that you're doing there. Now is there a way to get from the wifi network you can access given that it's an open network and access the network that all the business related operations takes place on that's locked without the typical wifi hacking. So I guess what I'm asking is. Is there a way to back track from the free wifi to the locked one.
Doctor's offices are a bit more secure, as they have to abide by HIPPA and probably a few other industry standards as well as state regulations. Either way going from public or an open wifi to private, it honestly depends for example if a vulnerable machine is multihomed and is exploited then yes it's possible. Let's say that they have a seperate router for the customers and for internal usage totally different even a second line dedicated to just business then no. There is a lot of unknown here, so in all truth I can't give a proper answer.
