I want that contact us form to middle of my website.
(I have tried with <div align="middle"> & <center> & <div align="center">)

Have you tried setting the margin like this?

Selector{
margin: 0 auto
};

Make sure you haven't set any position properties, you just want to have set the margin.

Anyways when it goes on like that and it gets the full-width you should just set on that div a smaller width.
