Honestly, this cover was the weaker one out of the ones they did. They had the meat puppets up and did two or three of their songs, and the "cover" they did of them was better than the originals. "Oh me" is tied with "Plateau". I can't decide which one is better for sure, but I'm definitely leaning toward Oh Me.

Nirvana kind of did something weird, they did lots of covers, when most bands at the time (not that I was there) did mostly their own material. That whole unplugged set was kind of a miracle since Kurt was coming down off a heroin high.

His guitar isn't distorted. He's using an electro acoustic. It uses the same idea of an electric guitar, but on an acoustic instead. Chorus? Yeah. He used it for CAYA. And distortion on the mics is also just plainly false. I'm not sure if it's because you don't know what you're talking about, or it's because you just didn't hear the unplugged set that I heard.

I think for anyone in a band using bass, they're at the least going to need to be plugged in. Them if anyone. And I doubt completely it was pre-recorded since MTV has published a few clips from the rehearsal they did before the actual show. That are (mostly) unedited. There's no reason for them to fake it. It'd be harder to fake than it would be to just... play... Keep in mind, Nirvana was a touring act (and practiced 5 days a week a lot of their career) for 6 years at that point. And Pat Smear (the guitarist behind Kurt) even longer than that.

I remember Kurt being on stage scoffing at the suggestion of "In Bloom". I think it would have worked. What's crazier is that Heart Shaped Box didn't make it. I think that it totally could have made it despite it being a hit.
