The etiquette states the course of a meal should go this way:
-Apetizer
- First dishes ( usually light dishes like soup, or small portions of pasta for example. Therefore that also means absolutely NO CHICKEN in the pasta, small chunks of meat for added taste is fine but absolutely NO MEAT balls.)
-Second dishes usually meat. ( steak and the liking, or chicken etc.)
-Fruit
And / or
- Dessert
Fruits and desserts can be sequenced or one may be excluded.
If the meal was particularly heavy a digestive shot accompanied by coffee is also expected.
Due to this classification Pizza is a Second and Pineapple is a last.
So it's viewed firstly as offensive for breaking rules that had been up for 500 years or so firstly and it's such a common practice that even who doesn't know the rules indeed perceives it as putting Ice Cream on a Hamburger and an indirect insult as to calling someone uneducated. Though the explanation is extremely simple it's because of this:

Inteligence is knowing a tomato is a fruit.
Wisdom is not putting a tomato in a fruit salad.
-Indeed you're right, tomatoes are fruits though they used like vegetables.
That I've always found peculiar too aswell as not native either.
I was always against pineapple on pizzas, but then I gave it a shot and I have to say... I was fucking right, it tasted horrible, but I guess some people like it so...

I agree as to why pineapple shouldn't be on Pizza but it's definitely not bad at all to eat lol
What about dishes about orange chicken? Surely the sweet tangy sauce combined with the protein-heavy chicken would be in violation of the same civil principles.
I understand where you're coming from and it's an interesting take, but I personally think it's more of the acidity of the tomato sauce that makes some people uncomfortable with adding a sweet topping. The texture of adding juicy toppings onto a generally firm pizza may also contribute.
Disclaimer: I am a fan of pineapple on pizza. Papa John's does it the best!
